VHCP Annual Reporting
When you are logged into the system, you will be able to submit your annual report by clicking the “My Agency” tab at the top of the page. Once you are on the “My Agency” page, at the very bottom, you can enter your annual report by clicking “Enter Annual Report.”
The VHCP Annual Report requires the following information:
Total number of Visits – VHCP does not provide guidance on how "Visits" should be defined or calculated. If possible, please use this definition from the Annual National Free & Charitable Clinics 2015 Survey conducted by Dr. Julie Darnell "Documented contact with a licensed healthcare provider constitutes a 'visit'. Visits performed by students under the direct supervision of licensed providers may be counted as a visit. Appointments carried out by dental providers or mental health/behavioral health providers are counted as a visit. Mental health/behavioral health includes services designed to promote well-being by preventing or intervening in mental illness, substance abuse, or other addictions. Exclude visits made for medication pick-up/refill, visits to determine eligibility for clinic services, visits for laboratory/diagnostic tests without contact with a licensed provider, and services provided off-site by referral providers". If you use this definition, please paste it in the comments section, so that when the data is exported we know which numbers are comparable.
Comments – The comment section will allow you to provide any feedback for the year including comments about your data. In particular, if you use the above "visit" definition, please paste it in the comments section, so that when the data is exported we know which numbers are comparable.
Patient – An individual who is receiving needed professional services that are directed by a licensed practitioner of the healing arts toward maintenance, improvement or protection of health or lessening of illness, disability or pain. (US Centers for Medicare & Medicaid Services, CMS)
New patient – An individual who has never before been seen by the clinic but who has subsequently had one or more visits this calendar year (2018).
Unduplicated patient – A patient with one or more contacts with the clinic during calendar year 2018. Unduplicated includes both new and returning patients. Unduplicated patients should only be counted once.
Services Provided – If you provide the same services, please confirm that you have the correct check boxed marked. If you have provided new services in the previous year, please check the appropriate boxes. (As this is the first annual report, please check the appropriate boxes – they will be carried into the following year on the next annual report).
Print/export a copy for your files. Should you need to edit your report after you have submitted it, please click “Send Request to Edit Annual Report” on the “My Agency” page.
