Seals of Excellence

There are three award levels of the Seals of Excellence. Each level requires a clinic to meet certain criteria for award of the specific seal. A clinic will be awarded the specific seal of interest for three years as long as the clinic maintains the requirements of the seal. WAFCC Seal of Excellence recipients demonstrate a commitment to promoting best practices and maximizing access to quality healthcare for the most vulnerable. ​

Gold Seal Recipients 
A clinic meets all “required” and “optimal” evidence as outlined in the Standards of Excellence.​
  • Community Outreach Heath Clinic (Menomonee Falls)
  • Muslim Community & Health Center (Milwaukee)
  • ​Health Care Network, Inc. (Racine)
  • ​Hope Clinic and Care Center (Menasha)
  • ​Open Arms Free Clinic (Elkhorn)
  • Lake Area Free Clinic (Oconomowoc)​
  • HealthNet of Rock County (Janesville)
Silver Seal Recipients
A clinic meets all “required” evidence as outlined in the Standards of Excellence.
  • Albrecht Free Clinic (West Bend)
  • Community Connections Free Clinic (Dodgeville)
  • ​InHealth Community Wellness Clinic (Boscobel)
  • Rock River Community Clinic (Jefferson)
  • Saturday Clinic for the Uninsured (Milwaukee)​
Bronze Seal Recipients
A clinic performs a  Standards of Excellence self-assessment and signs a Memorandum of Understanding (MOU) that outlines the clinic’s steps to achieve the Standards of Excellence. The MOU will include action items and timeline for completion. 
  • Neighborhood ​Free Health Clinic (Stoughton)
  • Waukesha Free Clinic (Waukesha)
  • Benevolent Specialist Project​ ​(Middleton)
  • St. Clare Health Mission ​(La Crosse)
  • SAM 25 (Shawano) 
  • MACH OneHealth (Madison)
  • ​First Presbyterian Church Wausau Free Clinic Free Clinic (Wausau) 
​A clinic can stay at the Bronze Seal level for a maximum of three years however, they can be resurveyed on a yearly basis. After three years a clinic must apply for the Silver or Gold Seal of Excellence and remove the Bronze Seal.
