Community Connections Free Clinic, Dodgeville

Community Connections Free Clinic (CCFC) provides primary health services to the uninsured and under-insured. 

VISTA Standards of Excellence Projects:
1. Research and implement fundraising database
2. Create and implement community awareness plan for services offered
3. Coordinate internal projects related to achieving Standards of Excellence. ​
4. Create and implement a social media communications plan for WAFCC

Lake Area Free Clinic, Oconomowoc

Lake Area Free Clinic (LAFC) has provided medical care to the uninsured, low income Waukesha County residents since 2001 - soon they will be adding dental!

VISTA Standards of Excellence projects:
1. Implement CareMessage
2. Update Volunteer Handbook
3. Create a Volunteer Satisfaction Survey
4. Standardize & Document Procedure for Giftworks
​5. Establish additional community partnerships and outreach for LAFC and WAFCC; foster community awareness on the safety-net and FCCs

Healthcare Network, Inc, Racine
​

Health Care Network, Inc. (HCN) provides free or lost health care to uninsured residents of Racine County with limited financial resources.

VISTA Standards of Excellence Projects:
​1.  Research and identify potential funding sources and strategic partnerships for HCN. 
​2. Create a fundraising plan.
3. Create and implement a media relations plan for HCN and WAFCC.​

Free Health Clinic, Stoughton

Free Health Clinic offers compassionate, non-emergency care free of charge to any adults in Stoughton and surrounding communities who are unable to afford the cost of health care.

VISTA Standards of Excellence Projects:
1. Develop a plan to improve the capability of the Free Health Clinic
2. Create procedures and forms to increase clinic sustainability
3. Develop volunteer training
4. Implement WAFCC's Standards of Excellence

HealthNet of Rock County, Janesville

HealthNet of Rock County, Inc. (HealthNet) provides medical and dental care to uninsured Rock County Residents who are at or below 200% of the FPL. 

VISTA Standards of Excellence Projects:
1. Create and implement a volunteer recruitment and management system
2. Write and implement a quality improvement plan
​3. Research and identify potential funding sources and strategic partnerships for HealthNet and WAFCC

​WAFCC VISTA Leader
​

The mission of the WAFCC is to support, strengthen, and advocate for the uniqueness of the Wisconsin free and charitable clinics, the patients they serve, and the communities with whom they partner. ​

VISTA Leader Projects:
1. Communicate regularly with VISTA members
2. Provide resources and support to VISTA Members
3. Foster support for the WAFCC VISTA programs
4. Organize professional development opportunities for VISTA Members
​5. Assist with collection of and editing of reports

SUBSTANCE / OPIATE ABUSE REDUCTION COORDINATORS
VISTA MEMBER HOST SITES

Substance/Opiate Abuse Reduction Coordinator VISTA Members will assist the safety-net/FCCs in their host-site areas with achieving WAFCC Standard of Excellence OPC1a "A Clinic recognizes the opiate addiction/overdose crisis in its community"

Juneau County Health Department,
​Mauston
The mission of Juneau County Health Department is “To protect and promote health and wellness within Juneau County.” 
​
VISTA Substance Abuse projects:
1. Collaborate with community partners to reduce youth access to prescription and illegal drugs with a focus on opioids
2. Conduct on-going prescription drug campaigns
3. Create community awareness and mobilization to prevent and address opioid and other drug use and misuse
4. Assure the S.A.F.E. coalition and their partners are implementing best practices for opioid prevention and treatment

Jefferson
Jefferson County Human Services Department's mission is to enhance the quality of life for individuals and families in Jefferson County.

​VISTA Substance Abuse Projects:
1. Collaborate with Jefferson County's Alcohol, Tobacco, and other Drug Addiction Prevention Coalition to raise community awareness
2. Identify and work to address housing barriers in Jefferson County, with a focus on those struggling with opiate addiction
3. Explore the implementation of Peer Recovery Coaches in Jefferson County
4. Develop collaborative opportunities to address substance use and abuse with safety-net providers

Winnebago County Health Department,
​Oshkosh
Winnebago County Health Department is a leader in creating a culture that optimizes health and wellness in our community and its mission is to lead change by providing services and building partnerships that strengthen the community. 

VISTA Substance Abuse Projects:
1. Raise community awareness around the opioid epidemic and substance use/misuse
2. Develop collaborative partnerships to address substance use/misuse
3. Expand the capacity of WCDAC to address opioid and other substance use/misuse
4. Increase resources to combat opioid, alcohol, and other substance use/misuse

Janesville
The Rock County Public Health Department (RCPHD) is the catalyst that cultivates a spirit of wellness and improves the quality of life by promoting healthier lifestyles and environments through collaborative community partnerships. 

​VISTA Substance Abuse Projects:
1. Raise awareness within the community around the opioid methamphetamine epidemic as well as other issues related to alcohol and other drug use/abuse
2. Increase capacities and competencies to combat the local drug epidemic
​3. Develop collaborative opportunities to address substance use and abuse where community members are
Walworth County Health Department,
​Elkhorn
The Walworth County Department of Health and Human Services (WCDHHS) exists to serve as a resource for persons who need assistance, a safety net for individuals and families in crisis, and a partner for improving quality of life. 

​VISTA Substance Abuse Projects:
1. Raise community awareness around substance abuse and the opioid epidemic
2. Increase resources to combat the local drug epidemic
3. Develop collaborative opportunities for community members to address substance use and abuse
